Teacher Survey Example - Adapted SNAP-IV Assessment

This example illustrates the ease of collecting important student data using a simple survey. Many students are admitted into treatment facilities having a history of ADHD features in school. A common objective for clinicians is to ascertain the intensity/frequency of the ADHD signs a student manifests before determining the proper diagnosis/approach. We administer this an adapted SNAP-IV assessment to teachers that takes an average of 2-3 minutes to take for each student we want to assess. 

You can download a template of this survey and use it to gather information from your teachers. You must have a functioning Google account, and once you click the 'Use this template' link, you may edit it and send it out to your academic staff immediately

Academic Wheel - Creating a robust student profile

Once your academic staff have a common assessment and reporting language, and are trained on the key clinical signs and symptoms you want to assess, use a collaborative tool to collect their observations into a polished report. This is an example of a report that can be used to identify the key areas of focus for a clients treatment.
